Report 16.26 Students with Disabilities – Initial Part C to B Transition Student List

Description: Reports details for students who are Eligible and Participating Students with Disabilities on an IFSP plan prior to 3rd birthday and who are either

1) overdue on Part B Initial Evaluation meeting as of the Report As Of Date,
2) Part B Initial Evaluation was held after the student’s 3rd birthday,
3) the Part B Initial Evaluation was held on-time, or
4) Part B Initial Evaluation Not yet held, student not yet 3 years old.

Note: Non-Compliant for the Annual Determination Letter (ADL) is defined as a Part B Initial Evaluation that is overdue and the Meeting Delay Code = 90, 40, or no Meeting Delay Code.

Meetings held late with or without a Meeting Delay Code, although technically non-compliant, are NOT included for C to B compliance in the ADL and will continue to appear on the report until the following Academic Year.

Basic selection criteria

  • Students and data included: Students with a qualifying enrollment who turn 3 years old during the academic year selected and have a PLAN record where Plan Type = 150 (IFSP) and Plan Effective Start Date is before the 3rd birthday.

Qualifying Enrollment:

  • Enrollment Status = Primary (10), Short-Term (30), Non-ADA (50)
  • Grade Level = IN, PS
  • Most recent Open Enrollment on the Report As Of Date.
  • Most recent = the last enrollment based on the last Student Enrollment End Date on or after the report period start date.
  • Students whose most recent SENR record on Report As Of Date is E155, E170, or E450 exit are considered qualifying enrollment for monitoring report purposes.
  • If more than one SENR record overlaps the report date:
  • Priority: Primary → Short-Term → Non-ADA
  • If more than one SENR record exists with the same Exit Date:
  • Use the top record in descending order by Start Date, Grade Level, and CDS.

Report will pull:

  • All PLAN records effective during and prior to the Academic Year selected (regardless of LEA). Includes PLANs after student’s 3rd birthday if effective during selected Academic Year.
  • All SWDS records effective during and prior to the Academic Year selected (regardless of LEA). Includes SWDS after the 3rd birthday if effective during selected Academic Year.
  • MEET records where Evaluation Type = 15 (Part C) and Referral Date is less than 90 days prior to SSID’s 3rd birthday.
  • All MEET records where Evaluation Type = 10 (Part B) and Pending As Of is populated or Meeting Date is populated.
  • Student Information (SINF), Student Program (SPRG), and Student English Language Acquisition (SELA) data used for student subgroups (Race/Ethnicity, Homeless, EL, etc.) from the most recent record as of the Report As Of Date.

Meeting Held Late: IFSP (150) start date before 3rd birthday; student age = 3; MEET Part B Meeting Date is after 3rd birthday.
Meeting Overdue With Pending Record: IFSP (150) start date before 3rd birthday; student age = 3; Pending Date exists; no Part B Meeting Date exists.
Meeting Overdue Without Pending Record: IFSP (150) start date before 3rd birthday; student age = 3; no Meeting or Pending Dates exist; SWDS = Eligible and Participating (1).
Timely Part B Transition: IFSP (150) start date before 3rd birthday AND (Part B Meeting Date ≤ 3rd birthday OR Part B Meeting Date > 3rd birthday AND Part C Referral Date < 90 days before 3rd birthday).
18 Non-Compliance Indicator (Y/N): Y if:
• Meeting Overdue Without Pending Record, OR
• Meeting Overdue With Pending Record AND Delay Code = 90, 40, or blank, OR
• Meeting Held Late AND Delay Code = 90, 40, or blank.
19 Days Overdue: Days since 3rd birthday.
Example: 3rd birthday = 5/1/24, Report Date = 5/25/24 → 24 days.
